1: WHERE IS THE OFFICE?
-Visit us at 1011 E. 151st Street Olathe, Kansas 66062

- We are located on the NW Corner of 151st and Mahaffie next to Goodyear/Burnett Automotive and Fuel Cafe.

2: HOW MUCH IS MY FIRST VISIT? 
-Typically for a new patient at our office the first visit is approximately $90

-That includes a new patient exam ($50) and treatment if appropriate

3: DO WE TAKE INSURANCE? 
-We accept all HSA and FSA insurance cards, all debit / credit cards, cash, cash app, apple pay, android pay or check.

-We currently are out of network with all private insurance companies.

-Itemized receipts available upon request

4: HOW DO I SCHEDULE A APPOINTMENT?
-Call or text us at 913-325-0720 

-We can digitally send you all necessary paperwork via our secure patient portal email link.

-All digital paperwork can be completed easily on any smartphone, tablet, or computer. 

-We also have paper intake forms available in office as well if digital paperwork doesn't work for you.
